Everything You Need To Know About Hirsutism

At our Miami med spa, we come across quite a few teens and women who clearly suffer from hirsutism (excessive hair growth in women). Years ago the school of thought was to send women who suffer from hirsutism to get their hormones checked out by an endocronologist. Unfortunately, 9 times out of 10, the tests came back as showing that nothing was wrong. What a shame, because most women who suffer from hirsutism, also suffer from poly-cystic ovarian syndrome. Clearly, something was wrong with some of our clients seeking facial laser hair removal treatments, because they had to shave their face because of the heavy hair growth. 

The question we asked time and time again was, how can a client that has such abnormal hair growth not have an issue with their hormones or endocrine system? The problem with most lab tests is that they are limited and do not always point towards hirsutism. Well, in most cases! The myth that if a lab test doesn’t positively identify a disease or health issue, than it doesn’t exist, is absolutely wrong. A negative result does not mean there is no problem, just that a particular disease is not present.

One thing is a fact- hirsutism is ALWAYS due to androgens. Androgens are commonly known as male hormones. However, before you get yourself into a panic, androgens are found in both men and women, just as estrogen is found in men. The difference is that after puberty, males have about 10 percent more androgens than their female counterparts. Testosterone is the most active androgen. The ovary and adrenal gland release testosterone. If the blood level of testosterone is elevated, it is important to find out whether it is coming from the ovary, the adrenal gland, or both.

Poly-cystic ovarian syndrome (PCOS) is usually the blanket diagnosis for the increased level of testosterone in women; however, hirsutism is not an accurate description of PCOS. PCOS includes signs and symptos such as: infrequent, often painful periods, failure of the ovary to release an egg cell, obesity and yes, hirsutism.

However, hirsutism is not only limited to young women, post-menopausal women may suffer from hirsutism as well. Thecal cell hyperplasia basically means that high testosterone levels are present in women pre or post-menopause. The theca cells are in the ovary and make testosterone. Theca cells lack an enzyme necessary to make estrogens themselves. It is believed that women who suffer from PCOS will also suffer from thecal cell hyperplasia. Although at this time, there are no known test to proove this theory.  "pcos diagram"

The reason why tests may come back with normal levels of androgens is because is hirsutism is not always caused by high levels of androgen in women, but by their actions. Is is possible, although not usually the case, to have high levels of androgen, and not have increased hair growth.

Women who suffer from hirsutism may have excessive hair growth around the nipples, abdomen, underarm, facial hair, genitals, as well as around the anus.  Increased testosterone levels in the adrenals and ovaries are just one explanation for hirsutism. If tests prove that your ovaries and adrenals are working fine but they still have too much hair, this can be confusing. Some women have follicles that are so sensitive to testosterone that even normal levels stimulate them.

Let’s go over the lab tests and what they should test for. First and foremost, tests should hopefully eiliminate diseases such as tumors and cancer or serious hormone imbalances. If tests prove that either the adrenals or the hormones are producing too much testosterone, there are medications that will control this imbalance. Common medications such as birth control pills, Aldactone, and topical creams such as Vaniqa may be prescribed. It may take a few months before you start seeing a difference.

We are not big fans of synthetic (lab produced) medications. We do recommend you seek hormone replacement treatments. Of course, laser hair removal is a long proven method for permanently reducing excess hair growth.
